Step 1: Choose Your Plan
Visit crateshealth.com and pick the plan that fits your needs:
One Product ($39/year) - 1 LMN, great if you have one specific purchase in mind
Three Products ($79/year) - 3 LMNs, covers a few wellness expenses
Unlimited ($99/year) - Unlimited LMNs, best value for maximizing savings
Remember: your Crates membership fee is itself HSA/FSA eligible!
Step 2: Create Your Account
Enter your basic information: - Name - Email address - Password
Step 3: Complete Your Health Assessment
This is a quick questionnaire (about 2-3 minutes) that asks about: - Your general health profile - Any conditions you manage or have been diagnosed with - Your current wellness routine (gym, supplements, wearables, etc.) - Products you'd like to cover with HSA/FSA funds
Why do we ask this? Our licensed healthcare providers use your health profile to determine which products are eligible and to issue your Letters of Medical Necessity. The more complete and accurate your information, the better we can help you.
Step 4: Select Your Products
Choose which wellness products and services you'd like LMNs for. You can browse categories like: - Gym memberships - Fitness trackers & wearables - Supplements & nutrition - Recovery tools - Sleep technology - And more
Step 5: Get Your LMN
Our licensed healthcare providers review your health information and issues your Letters of Medical Necessity. Many LMNs for catalog products are issued within minutes. For custom or non-catalog product requests, it typically takes 24-48 hours.
You'll receive a notification when your LMN is ready.
Using Crates for Your Family
You can use your HSA/FSA to cover eligible expenses for your spouse and dependents. Here's how it works:
For the health profile: Use the information of the person who needs the LMN (your spouse, child, etc.)
For the reimbursement: Enter the HSA/FSA account holder's information
For example, if your daughter needs a gym membership covered but your husband holds the HSA, you'd use your daughter's health info for the assessment and your husband's HSA details for reimbursement.
Account Tips
Keep your health profile updated - If your health situation changes, updating your profile may qualify you for additional products.
Check your email - We'll notify you when your LMN is ready and send important updates about your account.
