Quick answer: Crates Health can issue Letters of Medical Necessity (LMNs) for any HSA or FSA admin — no exceptions. For reimbursement support directly through our platform, we currently support HealthEquity, Fidelity, Optum, HSA Bank, Bank of America, WEX, Navia, and FSAFEDS. If your admin isn't on that list, you can still use Crates for your LMN and then submit for reimbursement to your HSA/FSA admin yourself.


LMN Support (All HSA/FSA Admins)

Crates can issue Letters of Medical Necessity for any HSA or FSA admin. It doesn't matter who your admin is — we've got you covered for LMNs.

Reimbursement Support (Select Admins)

For reimbursement support directly through the Crates platform, we currently support these admins:

With these admins, you can upload your receipt to Crates and submit for reimbursement directly from our platform — no need to log into your HSA/FSA admin's portal.


If Your Admin Has Direct Reimbursement Support

If your HSA/FSA admin is one of the eight listed above, here's the process:

  1. Take the Crates health survey

  2. We assess your eligibility for a particular product and health condition

  3. We provide you a personalized Letter of Medical Necessity

  4. Purchase the product

  5. Get your itemized receipt

  6. Upload the receipt to Crates

  7. Click "Start Reimbursement" directly through the Crates platform

We submit the documentation to your HSA/FSA admin on your behalf — you don't have to log into your admin's portal.

If Your Admin Isn't Listed

If you don't see your specific admin above, don't worry. We can issue LMNs for any HSA or FSA admin. Here's the process:

  1. Take the Crates health survey

  2. We assess your eligibility for a particular product and health condition

  3. We provide you a personalized Letter of Medical Necessity

  4. Purchase the product

  5. Download the LMN from the Crates platform

  6. Get your itemized receipt from the purchase

  7. Go to your HSA/FSA admin's portal

  8. Upload both the LMN and the receipt

  9. Submit for reimbursement through your admin's portal


Admin-Specific Tips

HealthEquity

Upload receipts and LMNs through their member portal or mobile app. Generally smooth process for wellness reimbursements.

Navia

Can be strict on what they consider "eligible products." Always submit your LMN alongside your receipt. If denied, ask for the specific reason and reach out to us for help.

FSAFEDS (Federal Employees)

Submit claims through the FSAFEDS portal. Federal employee FSAs follow standard IRS rules. LMNs are accepted for eligible wellness expenses.

Inspira Financial

Often requests itemized receipts (not just a credit card statement). Make sure your receipt clearly shows the product name and amount.

Optum

Note that Optum Financial and Optum Bank are technically different entities but serve similar functions. For FSA claims, submit through their claims portal.

Fidelity (HSA)

One of the simplest HSA admins. Keep your receipts and LMN for your own records in case of an IRS audit.


Frequently Asked Questions

Does Crates submit my claim for me, or do I submit it myself? It depends on your admin. For the eight admins we support for reimbursements (HealthEquity, Fidelity, Optum, HSA Bank, Bank of America, WEX, Navia, FSAFEDS), Crates can submit documentation on your behalf through our one-click reimbursement. For all other admins, you get your LMN from Crates and submit the claim to your admin yourself.

I'm with Cigna. Does Crates work with Cigna? Yes — we can issue LMNs for Cigna members. We're actively working on expanding our direct reimbursement integrations. For now, you'd take your LMN and receipt and submit to Cigna's portal yourself.

Can I switch HSA admins and still use Crates? Yes. Crates works independently of your HSA/FSA admin. If you switch admins, your Crates account and LMNs remain the same. You would just need to update your HSA admin information in the Crates platform, and then you'd be able to submit reimbursements directly from our platform as well.

My employer changed our FSA admin. Do I need to do anything on Crates? Yes. If you'd like to issue new Letters of Medical Necessity or process reimbursements, you'll need to update your FSA admin account information on the Crates platform. Once updated, you'll be able to issue LMNs and submit claims via our platform. That said, the existing LMNs you have and the purchases you already made remain valid for your previous FSA admin.
