Diagnosis Doesn't Match Your Condition
Your LMN should reflect the health conditions you reported in your health assessment. If the diagnosis listed doesn't match what you expected, let us know. This could mean your health profile needs updating, or there was an error in processing.
Missing or Wrong Products
If the LMN lists products you didn't request, or is missing products you need, we can get it updated. If you're on a plan that allows multiple LMNs, we can also issue additional ones for other products.
Important Notes
Don't submit an incorrect LMN to your HSA/FSA admin. An LMN with wrong information (especially a wrong name) will almost certainly be rejected. Wait for the corrected version before submitting your reimbursement claim.
If you've already submitted an incorrect LMN, let us know. We can provide the corrected version along with a note explaining the correction, which you can submit to your admin.
