Here's how to use Crates for a family member:
For the Health Assessment
Use the family member's information: - Their name - Their health profile and conditions - Their wellness products
The LMN needs to be for the person who will actually use the product.
For the Reimbursement
Use the HSA/FSA account holder's information: - The account holder's name - The account holder's HSA/FSA admin details - The bank information connected to the HSA/FSA portal

Health assessment: Enter your daughter's health info
LMN: Issued in your daughter's name for the gym membership

Each person needs their own LMN. If both you and your spouse want gym memberships covered, you'll each need a separate LMN with your individual health profiles.
The LMN must match the user. The person named on the LMN should be the person using the product or service.
Dependent children must be claimed on your taxes. You can only use HSA/FSA funds for dependents you actually claim on your tax return.

Can I cover my parents? Only if they're claimed as dependents on your tax return. Adult parents who file their own taxes generally can't be covered by your HSA/FSA.
